OUR HISTORY

MCAVHN Care and Prevention Network was established in 1987 as a volunteer AIDS support services provider to assist individuals who were struggling with HIV and AIDS, and required end-of-life supports and services. As the decades transformed the client base, the agency transformed the services offered, and began this endeavor by offering prevention services through outreach and engagement, eventually adding syringe exchange services in the local Ukiah areas, and later with the help of the California Department of Public Health/Office of AIDS and a sub-contract with MCHHSA, throughout the entire county.

MCAVHN serves persons at-risk or affected in multiple case management programs that have been adapted and developed over the past three decades. MCAVHN has expertise in supported housing (29 years), working with those who are mentally ill, substance-using; have chronic illness and health conditions, including those chronically unhoused. Our specialty and strength is making connections with persons experiencing co-occurring disorders. We have recently reinvested in our connections to tribal entities all over Mendocino County, and are establishing more relationships built on real trust; a major component of this development of trust being, that we follow through on our promises with respectful engagement.

MCAVHN utilizes best practices and adaptations from direct experience with multiple case management programs, including the California State Office of AIDS Medi-Cal Waiver program, the HRSA Ryan White Part B (HIV Case Management) program, Hepatitis C

Care Initiatives, Hepatitis C Advocacy Grants, the Health Outreach & Prevention Education (HOPE) program, CUSOC (Chronic Users System-of-Care), Behavioral Health Court; our more recent Whole Person Care project and Medi-Cal reimbursed Specialty Mental Health Care Management programs (all programs providing services for those with co-occurring disorders).

MCAVHN’s adaptation of case management services utilizes a holistic delivery system no matter the reason or place of entry in which individuals come for service. We have been addressing HIV for 34 years, those who are unhoused for 29 years, chronic hepatitis C for 18 years, “high utilizer” populations (more formally) for approximately 10 years through CUSOC (Chronic Users System of Care), ROSOC (Recovery-Oriented System of Care) and Whole Person Care, and since 2012, those with severe and persistent mental illness and co-occurring disorders. MCAVHN addresses the co-morbid conditions of mental illness, substance use, HIV, hepatitis C, other chronic health conditions experienced by persons experiencing homelessness; the gamut of conditions related to high-risk environments and subsequent behaviors since we began serving Mendocino County.
